- Past and Present Oral Pathologic conditions: Keratenized patch on left posterior buccal mucosa. Bilateral erythematic appearance of sclera. Apical lesion on #15 tooth.
- Missing (7) teeth and cause of teeth loss are due to caries, periodontitis, and trauma.
- Stage 3 & Grade B of Periodontitis
- Prognosis is Fair
- Periodontal Status at re-evaluation appointment improved due to decreased bleeding sites on probing.
- Long-term prognosis is poor.
