- Decide on a community agency to interview. To avoid duplications, kindly forward the name of the agency to instructor by listing the information on the Chat tab.
- Research and/or visit your community agency.
- Gather information, interview staff members from the community agency and collect resources like brochures and pamphlets to help you best understand their services. Any resources you collect are for you to keep.
- It is optional to interview someone from the community agency resource but encouraged for assistance to you with the following information to help you create a one page Fact Sheet. Fact Sheet Contents Must Include the Following:
- Name, address and phone/fax numbers
- Hours of service; Days of Week
- Area served, boundaries (limits of service)
- Who’s eligible for these services (ages, etc.)
- Type of services they provide
- Fees Charged
- Person(s) in charge of services
