Community Survey Project (Social Determinants of Health & Health Promotion): The
team of students will survey the assigned community and compile a report as described in the Independent Study Project in the syllabus.
1. Each virtual clinical group will choose a “community”.
2. This is an obstetrics and pediatric focus paper. The project must speak to how social
determinants of health and health disparities affect the health and well-being of the
maternal-child population.

4. The student will search the databases of NYS DOH, NYC DOH, And CDC websites
for factors that contribute to health disparities and accessibility to healthcare,
determine a cause, and compile a report for submission during the final week of this

5. As a group, your report will be a written report of the survey to your instructor. Use the Learning Outcomes as your guide to structure your report (See rubric)
6. Discuss your experiences in searching specific neighborhoods and your findings.
