Design a documentation chart that can be used by service providers to record and share information about their interactions and the services provided to students during the school day. The design of the documentation chart should be compliant the laws and policies that govern the management of confidential student information and should include space to document the following:
- Service provider name, title, date of service, and summary of services provided. Include space for a speech pathologist, occupational therapist, physical therapist, and two additional related service providers of your choice.
- Specific area for documenting student progress related to IEP goals.
- Specific areas for documenting general observations, information, questions, and concerns about the student.
In 250-500 words, summarize how you will collaborate and communicate with related service providers to ensure student needs are being met by addressing the following:
- Describe the primary role of each service provider (speech pathologist, occupational therapist, physical therapist, and two additional related service providers of your choice) including a summary of the services typically provided to the students.
- Explain how the support/therapy/activity provided by each service provider can assist students in meeting IEP goals.
- Describe two specific strategies you will employ in your future professional practice to collaborate and communicate with related service providers to ensure accuracy of shared information and a classroom environment and experiences that maximize direct instruction and learning.
Support the assignment with a minimum of three scholarly resources.
