Please write a care plan report for a patient (using a pseudonym) with a complex health condition related to gerontology nursing.The report must provide an overview and background of the patient’s current health problem/s that contextualises the need for a person-centred, family care approach to care. The care plan must be evidence-based and demonstrate critical thinking, analysis, synthesis and evaluation of the literature to support the management and holistic care needs chosen for the patient.
