Specifically, you will share what you perceived to be a suitable opportunity to establish a public-private partnership between the Grant City Health and Human Services and hospital\/medical center with supporting rationale.<\/p>\n<p><i><strong>Getting to Know Your Local Health Department<\/strong><\/i><\/p>\n<p><i>A representative local health department called Grant City Health and Human Services exists in a virtual community known as Grant City. For this Discussion, you will explore the resources, programs, and personnel of an existing local health department using the information available within the Grant City Interactive. You will become more familiar with Grant City later in this module and again later in the course. <\/i><\/p>\n<ul>\n<li>Review the following case study scenario.\n<ul>\n<li><em>You work for the local health department, and your Director, Dr. Dana Foster, is looking for an opportunity to establish a local partnership that shares resources to meet a public health need in the community.<\/em><br \/><em>The Patient Protection and Affordable Care Act of 2010, 26 U.S.C. \u00c2\u00a7 501(r), requires all 501(c)(3) hospitals and medical centers to conduct a comprehensive community health needs assessment (CHNA) every 3 years; to prepare a report of findings that is easily understood by the general public; and to be accessible on the company\u00e2\u20ac\u2122s website. There are a growing number of local health departments that are working with non-profit hospitals and medical centers to complete a comprehensive CHNA in their communities.<\/em><br \/><em>As a member of the local health department, you assisted in the completion of one of the local hospital\u00e2\u20ac\u2122s\/ medical center\u00e2\u20ac\u2122s CHNA, which has been posted on their website for ease of public access. Dr. Foster is eager to move beyond assisting with the development of the CHNA and is now looking to use the information obtained from this completed assessment to build a collaborative partnership with the local hospital\/medical center to identify and address a public health problem. The leadership team of the local hospital\/medical center is equally eager to enter into a partnership, assuming the right opportunity presents itself. To this end, Dr. Foster has asked you to examine the most recent CHNA to see if there is a suitable opportunity to forge a public-private partnership to address a public health problem. What are the strengths of interdisciplinary teams in public health? Why is it necessary to include multiple professions and various systems in public health?<\/p>\n<ul>\n<li><strong>Note<\/strong>: Examples of professions that may be integrated are physicians, x-ray technicians, respiratory therapists, registered dieticians, physical therapist, etc. The professions will vary by health issues and clinic\/hospital in the community.<\/li>\n<\/ul>\n<\/li>\n<li>Locate at least two scholarly resources, less than 5 years old, related to the use of interdisciplinary teams to improve public health.<\/li>\n<\/ul>\n<p><i><\/i><\/p>\n<p><strong>HINT<\/strong>: Use the Table of Contents in the CHNA to locate relevant information quickly. Consider reviewing the staffing and leadership structure, populations served by the hospital, services offered to the community through the hospital, and the specific programs offered by the hospital that would be a good match for your health department.<\/p>\n<h3>PART 1: 1-2-PAGE PAPER<\/h3>\n<p><strong>Prepare <\/strong>a 1-2-page paper in which you:<\/p>\n<ul>\n<li>Identify a non-profit hospital or medical center in a local community that has a publicly available CHNA. Use this as a starting point for identifying a suitable public health issue and a potential public-private partnership.<\/li>\n<li>Identify a public health problem that can be addressed through a potential partnership between health systems, providing evidence of the public health problem impacting the community and that warrants an intervention.<\/li>\n<li>Identify at least three potential professions from both your selected CHNA and <a class=\"inline_disabled external\" href=\"https:\/\/cdn-media.waldenu.edu\/2dett4d\/Walden\/PUBH\/8317\/GrantCity\/index.html\" target=\"_blank\" rel=\"noreferrer noopener\">Grant City<\/a><\/li>\n<li>Health and Human Services that could be utilized for a successful collaboration. Include the strengths of interdisciplinary teams in public health and why it is necessary to include multiple professions and various systems in public health.<\/li>\n<li>Explain how the potential partnership aligns with and supports the mission (or purpose) statement of each organization. Remember, the vision, mission, and strategic goals of an organization reflect its strategic direction.<\/li>\n<li>Describe what this partnership might look like in terms of addressing the identified public health problem. In other words, what would the intervention look like that will address this public health problem?<\/li>\n<li>Support your response with at least two scholarly resources, less than 5 years old. In addition, include the source of the community CHNA used in your analysis of collaborative partnerships. Properly cite\/reference using APA 7th edition.<\/li>\n<li>\n<h3>PART 2: 1-PAGE BUSINESS EMAIL<\/h3>\n<ul>\n<li><strong>Draft <\/strong>a business email (limited to 1 page in length) for Dr. Foster. Remember, Dr. Foster intends to send your email to the director of the local medical facility to encourage collaboration and the fostering of a partnership to address a public health problem.<\/li>\n<li>Your business email should convey a persuasive argument to integrate knowledge, approaches, methods, values and potential contributions from multiple professions and systems in addressing public health problems.
