In this assessment, you gather information for a family assessment and look at the medical history and social influences on a family unit. You will conduct a windshield survey of the community in which your chosen family resides.<\/p>\n<p><strong>Part 1: Family Assessment<\/strong><\/p>\n<p><strong>Select <\/strong>a family within your community as the basis for the completion of assessments in this course.&nbsp;The family must have at least 1 child under the age of 18 living the home.<\/p>\n<p><strong>Complete <\/strong>and <strong>submit <\/strong>a signed Family Release of Confidentiality form to your course instructor prior to your first point of contact with the selected family.&nbsp;<\/p>\n<p><strong>Review <\/strong>the key points of therapeutic conversation.&nbsp;<\/p>\n<p><strong>Formulate <\/strong>key questions for obtaining valuable information.&nbsp;<\/p>\n<p><strong>Ensure <\/strong>your family assessment addresses:&nbsp;<\/p>\n<ul>\n<li>Identifying data&nbsp;<\/li>\n<li>Developmental stage and history of family&nbsp;<\/li>\n<li>Environmental data&nbsp;<\/li>\n<li>Family structure&nbsp;<\/li>\n<li>Family functions&nbsp;<\/li>\n<li>Family stress and coping&nbsp;<\/li>\n<li>Family composition<\/li>\n<li>Parenting styles<\/li>\n<\/ul>\n<p><strong>Provide <\/strong>a brief overview of the family members.&nbsp;<\/p>\n<p><strong>Define <\/strong>child abuse and neglect using your state\u2019s terminology. Note any parenting weaknesses that might indicate possible abuse and\/or neglect.<\/p>\n<p><strong>Write <\/strong>a minimum 875-word summary of Part 1.<\/p>\n<p><em>Note<\/em>: Remove all personally identifying information, such as the family members\u2019 names. Refer to family members by initials only.&nbsp;<\/p>\n<p><strong>Part 2: Community Assessment&nbsp;<\/strong><\/p>\n<p><strong>Review <\/strong>the <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/myresource.phoenix.edu\/secure\/resource\/NSG482v4\/nsg482_v4_windshield_survey_aid.docx\">Windshield Survey Aid<\/a>.&nbsp;<\/p>\n<p><strong>Review the Windshield Survey Resources&nbsp;<\/strong><\/p>\n<p><strong>Observe <\/strong>your family\u2019s community by driving around the area.&nbsp;<\/p>\n<p><strong>Consider <\/strong>aspects of the community that could affect residents&#8217; health and any <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/health.gov\/healthypeople\/objectives-and-data\/leading-health-indicators\">HealthyPeople Leading Health Indicators<\/a> that may be applicable to the community.&nbsp;<\/p>\n<p><strong>Conduct <\/strong>a <strong>windshield survey <\/strong>that addresses the following components:&nbsp;<\/p>\n<ul>\n<li>The age, nature, and condition of the community\u2019s available housing&nbsp;<\/li>\n<li>Infrastructure needs\u2014roads, bridges, streetlights, and so on&nbsp;<\/li>\n<li>The presence or absence of functioning businesses and industrial facilities&nbsp;<\/li>\n<li>The location, condition, and use of public spaces&nbsp;<\/li>\n<li>The amount of activity on the streets at various times of the day, week, or year&nbsp;<\/li>\n<li>The noise level in various parts of the community&nbsp;<\/li>\n<li>The amount and movement of traffic at various times of day&nbsp;<\/li>\n<li>The location and condition of public buildings\u2014the city or town hall, courthouse, and so on&nbsp;<\/li>\n<li>Walkability of community: walking paths\/biking paths\/safety and proximity for walkability to community resources<\/li>\n<li>Public transportation availability, cost, accessibility<\/li>\n<li>Social service centers, such as public libraries, parks, churches, community centers, recreation centers&nbsp;<\/li>\n<li>Health care facilities, including how many, such as clinics and hospitals, and accessibility of health care resources<\/li>\n<li>Food\/nutrition, including details such as food deserts, grocery stores&nbsp;<\/li>\n<li>Homeless population and homeless services details&nbsp;<\/li>\n<li>Police and fire department presence&nbsp;<\/li>\n<li>Schools and their location, including physical condition of schools, and note whether daycare, preschool, elementary, middle school, high school.<\/li>\n<li>Community resources related to child abuse and neglect, spousal abuse, or elder abuse<\/li>\n<\/ul>\n<p><strong>Compile <\/strong>your windshield survey data, using the <a rel=\"nofollow noopener\" target=\"_blank\" href=\"https:\/\/myresource.phoenix.edu\/secure\/resource\/NSG482v4\/nsg482_v4_wk2_windshield_survey_template.docx\">Windshield Survey Template<\/a> to include:&nbsp;<\/p>\n<ul>\n<li>Data overview of all community components as identified in the Community Tool Box Resource&nbsp;(in the Windshield Survey Resources below)<\/li>\n<li>Strengths of community as evidenced by windshield survey&nbsp;<\/li>\n<li>Weaknesses (gaps in service) as evidenced by windshield survey&nbsp;<\/li>\n<li>Develop 1 family nursing diagnosis based upon identified gap in community resources as an indicator of potential poor health outcomes<\/li>\n<li>Develop 1 family diagnosis based upon identified gap in community resources as an indicator of potential poor\/weak parenting<\/li>\n<\/ul>\n<p><strong>Format <\/strong>your assessment as noted:&nbsp;<\/p>\n<ul>\n<li>18- to 20-slide presentation&nbsp;<\/li>\n<li>The slides should only contain essential information and as little text as possible.
