On Monday January 4<\/span><sup style=\"cursor: auto; color: inherit;\"><span style=\"font-size: 8pt; cursor: auto; color: inherit;\">th<\/span><\/sup><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">, 2021, James reported to work as usual at 7:00am. On this<br \/>\nparticular, he was working with his RN colleague Melissa, who had to take a double<br \/>\nshift because Richard (the RN who typically works Mondays) was ill. At roughly<br \/>\n8:30am, a patient by the name of Charles Becker entered the ER, grasping his<br \/>\nright shoulder. He approached James\u2019 desk.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cHi<br \/>\nthere,\u201d remarked Charles, who was seemingly in pain.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cHi.<br \/>\nWhat brings you to the ER?\u201d asked James.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">Charles<br \/>\nexplained that he was involved in an accident at home whereby he fell from his<br \/>\nporch and landed on his shoulder, which he thinks is now dislocated or broken.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cOk,<br \/>\nlet me take your temperature and blood pressure,\u201d said James, preparing the<br \/>\ninstruments.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cNurse,<br \/>\ncan I please have some Tylenol or something. It really hurts,\u201d inquired<br \/>\nCharles.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cI<br \/>\nwill go double check with the doctor,\u201d assured James. \u201cNo fever, so that\u2019s<br \/>\ngood. Your blood pressure will show in a second\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">Just<br \/>\nthen, Melissa joins them in the room, hunting intently for her eyeglasses that<br \/>\nshe left there earlier.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cHey<br \/>\nMelissa,\u201d says James, \u201cdo me a favour and record Charles\u2019 blood pressure. I<br \/>\nhave to quickly ask the doctor something.\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">Rushed,<br \/>\nMelissa notes 133\/88. It actually read, 188\/98. Normal blood pressure is<br \/>\nroughly 120\/80. She then remembers leaving her glasses in the bathroom when she<br \/>\nwashed her face.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cHey<br \/>\nDoctor Fleming,\u201d says James. \u201cQuick question for you. There\u2019s a-\u201c<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cMr.<br \/>\nJames,\u201d Doctor Fleming interrupts, \u201cdo you not see that I am reading an MRI<br \/>\nreport? As a nurse, you should recognize the importance of not distracting us<br \/>\nphysicians when doing important duties such as these.\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">James<br \/>\nis furious and embarrassed at the same time. He remains silent and begins to<br \/>\nwalk away.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cWait,\u201d<br \/>\nDoctor Fleming remarks. \u201cWhat is it now that you are here?\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cAn<br \/>\nER patient is requesting medication for pain. Looks like he dislocated his<br \/>\nshoulder. Can I give him anything in the meantime?\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cAcetaminophen<br \/>\nis fine James,\u201d responded the doctor. \u201cI am going to go speak with the<br \/>\nradiologist upstairs for a minute. I\u2019ll be back in a few.\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">James<br \/>\nreturned to the triage area. He gave Charles two extra strength Tylenol caplets<br \/>\nand a bottle of water. Charles returned to the waiting room.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">After<br \/>\n15 minutes, Melissa calls out, \u201cCharles, you are next.\u201d Charles rises from his<br \/>\nchair and quickly collapses to the ground. He begins to violently seizure.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">Melissa<br \/>\nruns towards Charles, calling out \u201cJames, get over here please.\u201d James joins<br \/>\nMelissa with some medical instruments to attempt to settle Charles.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cWhat<br \/>\nthe hell happened?\u201d James claims, as he straps the blood pressure machine cuff<br \/>\non Charles. \u201c195\/125! He is in hypertensive crisis! How can this be? Melissa,<br \/>\nwhat was his bp earlier?\u201d<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">Melissa<br \/>\nresponds, \u201cI don\u2019t remember exactly, but something stable like 130\/85. Wait,<br \/>\nlet me just check the machine\u2019s memory data.\u201d 188\/98.<\/span><\/p>\n<p style=\"line-height: normal; cursor: auto; color: inherit;\"><span style=\"font-size: 10.5pt; cursor: auto; color: inherit;\">\u201cIf<br \/>\nit was that high to begin with, then\u2026 oh no\u2026 the Tylenol! Acetaminophen will<br \/>\nincrease blood pressure,\u201d James stated with fear in his voice. He went upstairs to radiology. We don\u2019t have time Melissa.
